Select Claims from the top menu and then select Complete a Claim from the sub-menu on the left.  Follow the steps to complete the claim as described under

Capturing of Exit Notifications

Complete a Claim

 

From the Benefit Claim Details screen, select Disability > Disability Details from the sub-menu on the left.  The lower portion of the screen expands to display Additional Details Needed for Disability Claim.

 

 

Illness Type

Click the LIST button for Illness Type and select a value from the list.

 

 

Medical Practitioner

Click the LIST button for Medical Practitioner and select a value from the list.

 

 

Hospital / Clinic

Click the LIST button for Hospital / Clinic and select a value from the list.

 

 

 

Once all the details have been captured, click SUBMIT.
