Deaths

This report displays all deaths logged in a specified period. 

 

From the Exits menu, click  alongside Deaths on the sub-menu on the left.  Additional options will be displayed.

 

 

New

Click New below Deaths.  The Deaths screen will be displayed.

 

 

To define the period for which deaths must be reported on, capture a From Date and a To Date.  Capture your e-mail address and click GENERATE REPORT.

 

The current report detail will be displayed on the lower portion of the screen.

 

 

A message will indicate once the job has been successfully submitted, and an e-mail will be sent to you.

 

View

Click View below Deaths.  The Report Selection screen will be displayed.

 

The following columns are displayed:

-        Scheme Code

-        Timestamp

-        Date captured

-        Report Type

-        Report Seq No

-        Action

 

To select a report, click on the hyperlinked display in the Action column alongside the appropriate report.

 

The Report Selection screen will be displayed.

-        Member number

-        Title

-        Forename

-        Preferred name

-        Surname

-        Age at Death

-        Date of Death

-        Address Line 1

-        Address Line 2

-        Address Line 3
